Scoring notes
Score what you observe, not what the patient reports. A lower total means higher risk — the friction/shear subscale only goes to 3, so a “perfect” score is 23, not 24.
Reassess on admission, per facility schedule, and with any change in condition. A single low subscale (e.g., constant moisture) warrants targeted intervention even when the total looks safe.
